This care model brings together multiple family physicians working collaboratively to deliver coordinated longitudinal and patient centred primary care. By functioning as a unified team our physicians can better support continuity share clinical expertise and ensure patients receive comprehensive evidence based care.
📍 Two Locations Serving the Guelph Community
To further improve access to care the East Guelph Family Health Group will operate across two locations in Guelph
🏥 East Guelph Medical Clinic (EGMC)
Our established east end clinic continuing to provide comprehensive family medicine services.
🏥 West Guelph Medical Clinic (WGMC)
Opening April 2026 this new west end location will expand access to primary care for patients across the city.
This two location approach allows us to serve a growing community while maintaining the same trusted standard of care at both sites.
