Your first appointment

Longer than a normal visit, because it is the one that sets everything up. Bring your health card and a list of your medications. We will request your records from your previous physician.

The same doctor at every visit, holding your history rather than reading it fresh each time.

Nursing

Immunizations, wound care, infusions, blood pressure checks and the monitoring that keeps chronic conditions steady.

Phlebotomy

Bloodwork drawn on site, so a requisition does not mean a second trip across the city.

Reception

Booking, referrals, records, forms and fees. The fastest route to an answer is usually the phone.

Enrolling

Can I join the practice?

You live in or near Guelph

We enrol across the city and the surrounding townships. If you are moving here, apply before you arrive.

You have Ontario coverage

A valid health card, or one you are in the middle of renewing. Reception will help if it has lapsed.

You are leaving another practice

Bring the name of your previous physician. We request the records; you do not have to chase them.

Enrolment is open

Start with the form. We call you within a week.
